Illinois C.A.R.E.S.™ Predictive Wellness Model
The Illinois C.A.R.E.S.™ Predictive Wellness Model looks upstream—before a crisis, emergency, or system involvement occurs. It recognizes that serious outcomes rarely happen in isolation. They often develop through a combination of social, environmental, behavioral, family, and resource-related factors. By identifying these factors earlier, we can better understand the pathways that may place individuals, families, and communities at increased risk.
The model moves beyond waiting for a crisis to occur. It focuses on Identify Risk → Understand the Pathway → Strengthen Protective Factors. Illinois C.A.R.E.S.™ brings together information about conditions such as housing, transportation, education, employment, healthcare access, family stability, community environment, and other social determinants of health to identify opportunities for earlier support.
The goal is not to predict a person’s future—it is to recognize risk early enough to help change the pathway
Through education, resources, collaboration, and evidence-informed strategies, the Predictive Wellness Model creates opportunities to intervene upstream and strengthen the conditions that support healthier individuals, families, and communities.
